When they book, patients are asked to fill in their health history form, which is customized with your branding and question options. Like everything we do, the form is mobile-optimized – and when completed, the information is stored on the patient’s profile for next time. Any important notes or allergies will flag on the schedule for that patient so nothing’s ever missed.
